Make sure to bind 'Reload Config' at the top under General (I use F1) if you're making these changes while the game is open. Try creating a profile for your controller in the GUI (DarkSoulsInputCustomizerGUI.exe - it'll be where you installed the mod, typically 'Local Disc/Program Files (x86)/steamapps/common/Dark Souls Prepare To Die Edition/DATA').In the GUI there should be a profile for XBox controllers there by default, but if not: under Basic Movement change 'Movement/Camera' Control to Gamepad and then bind your buttons accordingly.
Everything seems to work except the last mod (the Customizer). Note: I also tried the line by renaming dinput8dllWrapper to dinputdllWrapper DarkSoulsInputCustomizer.dll (eliminating the 8 in the first part). Changed the dinput8dllWrapper none to dinput8dllWrapper DarkSoulsInputCustomizer.dll.
