Try creating a profile for your controller in the GUI (DarkSoulsInputCustomizerGUI.exe - it'll be where you installed the mod, typically 'Local Disc/Program Files (x86)/steamapps/common/Dark Souls Prepare To Die Edition/DATA').In the GUI there should be a profile for XBox controllers there by default, but if not: under Basic Movement change 'Movement/Camera' Control to Gamepad and then bind your buttons accordingly.
